Referring Patients for Testing or Care

If you are calling from a UCMC physician's office and would like to refer your patient to the ROLD clinic, please see below.

For referral via PHONE:

Please call (773) 702-0880. We will ask for: a) the patient's name and contact information, b) your name and contact information, and c) the reason for referral.

For referral via FAX:

Please fax a referral letter to:
