The Asthma and COPD Center helps patients quit smoking. Because quit rates are highest when medications are combined with behavioral interventions, we offer patients a comprehensive strategy that includes both nicotine replacement therapy (NRT) and counseling. To refer a UCMC patient, please complete and fax the "Tobacco Cessation Program Fax Referral Form" to 773-834-0242. We will notify you once the patient has been contacted or if for any reason we are unable to provide assistance, via the following Referral Response Form. At this time, the Tobacco Cessation Program is limited to patients at the UCMC.
